BOARD OF COUNTY COMMISSIONERS <br />COUNTY OF KITTITAS <br />STATE OF WASHINGTON <br />RESOLUTION <br />NO. 2018-/5 &:J <br />RESOLUTION TO AUTHORIZE EXECUTION OF AN AMENDMENT TO THE <br />PROFESSIONAL SERVICES AGREEMENT BETWEEN YOUTH SERVICES AND <br />KITTITAS COUNTY PUBLIC HEAL TH <br />WHEREAS, Kittitas County, through the Kittitas County Public Health Department is charged <br />with the preservation, promotion, and improvement of health in the County; and <br />WHEREAS, the Kittitas County Public Health Department (KCPHD) and Youth Services of <br />Kittitas County (Youth Services) entered into an Agreement on May 9, 2018, for <br />the purpose of enlisting Youth Services to implement the Community Prevention <br />Wellness Initiative (CPWI) community based mentoring program in Kittitas <br />County; and <br />WHEREAS, the terms and conditions of said Agreement provide that the parties may mutually <br />amend the agreement in writing; and <br />WHEREAS, the Kittitas County Public Health Department and Youth Services desire to amend <br />the Agreement by making adjustments to the compensation allocations listed in the <br />Exhibit B matrix, the total amount of compensation remains the same but will be <br />applied differently and in the manner that is attached hereto and incorporated by <br />reference, which shall replace the original Exhibit B matrix in its entirety; <br />NOW THEREFORE, BE IT RESOLVED that the Board of County Commissioners of Kittitas <br />County, Washington, authorizes the Director of the Kittitas County Public Health <br />Department to execute the amended Agreement with Youth Services that is <br />attached hereto, and incorporated herein by reference. <br />DATED this {311-A day of &twh , 2018, at Ellensburg, Washington. <br />OF COUNTY COMMISSIONERS <br />S C " , WASHINGTON <br />Vice-Chair ~--- <br />Commissioner <br />ATTEST:
