RECIPIENT/CONTRACTOR agrees to keep proof in its agreement file, that it, and all lower tier recipients or <br />contractors, are not suspended or debarred, and will make this proof available to ECOLOGY before requests for <br />reimbursements will be approved for payment. FEDE RA L FUNDING ACCOUN T A B ILrrY AND TRAN PARENCY ACT {fF AT A) REPORl1lNG <br />REOC J IREM E TS : <br />CONTRACTOR/RECIPIENT must complete the FFATA Data Collection Form (ECY 070-395) and return it <br />with the signed agreement to ECOLOGY. <br />Any CONTRACTOR/RECIPIENT that meets each of the criteria below must report compensation for its five <br />top executives using the FFATA Data Collection Form. <br />• Receives more than $25,000 in federal funds under this award . <br />• Receives more than 80 percent of its annual gross revenues from federal funds. <br />• Receives more than $25,000,000 in annual federal funds. <br />Ecology will not pay any invoices until it has received a completed and signed FF AT A Data Collection Form. <br />Ecology is required to report the FF AT A information for federally funded agreements, including the required <br />DUNS number, at www .fs.rs.go <http ,//www.f r .gov/> within 30 days of agreement signature.
