PERSONAL SERVICES AGREEMENT ATTACHMENT 11 811 COMPENSATION THE COUNTY WILL NOT PROCESS PAYMENT FOR SERVICES RENDERED UNDER THIS AGREEMENT UNTIL THE CONTRACTOR SUBMITS A COMPLETED W-9 (SEE ATTACHMENT "F") As full compensation for satisfactory performance of the work described In Attachment "A", the County shall pay to the Contractor compensation not to exceed The Contractor shall submit monthly invoices to the County Project Manager listed on page 1 of this Agreement. The Contractor shall submit with each monthly invoice a written progress report to the County Project Manager, The progress reports shall include, at a minimum, the following: any problems, delays or adverse conditions which will materially affect the Contractor's ablllty to meet project objectives or time schedules together with a statement of action taken or proposed to resolve the situation. Reports shall also include recommendations for changes to the Scope of Work, If any. Payments may be withheld if reports are not submitted. 1. FEE SOfEOUL ✓ County will promptlv notify Consultant of anv revisions or amendments to County Fee Schedule ✓ County wlll forward a copy of revised or amended Fee Schedule to Consultant ✓ Consultant fees for Services provided pursuant to this Agreement will be as follows: Service Fee Schedule: Inspection Services •$100.00 per hour-four 14} hour minimum Re-Inspection Services •$100.00 per hour-four (4) hour minimum •Hourly rate for Inspections of $100.00 per hour will remain In effect for the initial twenty-four (24) month period of this Agreement. Subsequently, the hourly rate wlll Increase by 3% every twenty-four (24) months. •11me tracked wlll start when Consultant check In at the County .
