EXHIBIT "A" <br />SCOPE OF WORK <br />Contractor shall provide all material and labor to perform medical imaging for <br />Tuberculosis screening consistent with standard radiological practices for patients <br />referred to Contractor by the Kittitas County Public Health Department. Referrals for <br />medical imaging for Tuberculosis screening shall be made to Contractor using the <br />referral form generated by Kittitas County Public Health Department's electronic medical <br />record. <br />Such imaging shall include either a single -view or double -view digital medical image <br />with Contractor's picture archiving and communication system (PACS). <br />Upon completion of a medical image, Contractor shall provide access to the digital <br />image to the Kittitas County Public Health Officer, or any designee of the County so <br />communicated in writing to Contractor. <br />Professional Services Agreement <br />Page 13 <br />
