ATTACHMENT "A" <br />SCOPE OF WORK <br />Funding is provided for the following activities and deliverables: <br />1. Administer the Ages and Stages Questionnaire (ASQ) to aid in early identification of <br />children with developmental disabilities. <br />a. Deliverable: Through June 30, 2018, maintain participation in the ASQ program <br />at an average of twenty children throughout the contract year. <br />b. Deliverable: Through June 30, 2018, provide the County with quarterly reports <br />summarizing the number of children participating in the ASQ program and the <br />number of referrals generated to the Birth to Three program . <br />2. Conduct outreach activities with primary referral sources to provide support, <br />information , and assistance to families and children with developmental disabilities . <br />a. Deliverable: Through June 30 , 2018, conduct quarterly outreach activities with <br />primary referral sources. <br />3. Enhance direct services by purchasing appropriate curriculum, tools, therapeutic toys <br />and manipulatives for early intervention staff to utilize during home visits and <br />maintained in the lending library. <br />a. Deliverable: By June 30, 2018, provide a report to the County describing the <br />curriculum, therapeutic toys and manipulatives that are purchased for staff and <br />children participating in the Birth to Three program. <br />4. Provide ongoing developmental screenings, outreach and referrals at Early Leaming <br />Coalition, STEM events and other community events. <br />a. Deliverable: By June 30, 2018, attend a minimum of 15 community events <br />where staff provides developmental screenings, outreach and referrals. <br />5. Provide staff training to improve services to children with developmental disabilities . <br />a. Deliverable: By June 30, 2018, three staff in the ESIT program will obtain a <br />minimum of sixteen hours of professional development training each. <br />6. Provide quarterly reports to the County. <br />a. Quarterly reports to the County shall be due by the following dates: <br />i. October 15, 2017 (for July 1-September 30, 2017) <br />ii. January 15, 2018 (for October 1-December 31, 2017) <br />iii. April 15, 2018 (for January 1-March 31, 2018) <br />iv. July 15, 2018 (for April 1-June 30, 2018) <br />b. Quarterly reports shall include, at a minimum: <br />i. A narrative of progress on the deliverables outlined above. <br />ii. Number of children participating in the ASQ program. <br />iii. Number of ASQ participants referred to the Birth to Three <br />program. <br />iv. Number of outreach activities with primary referral sources. <br />Professional Services Agreement <br />Page 13
