ATTACHMENT "A" SCOPE OF WORK Funding not to exceed $18,000 is provided for the following Technical Assistance activities and deliverables: 1. WISE and/or a third party consultant shall provide training and technical assistance to employment providers and community members, including families, employers, Developmental Disability Advisory Committee members, school personnel, and/or other interested parties. Technical Assistance training shall focus on the following Employment Development Activities: • Provide training to employment providers on job development, job coaching, and transitioning clients from a group supported setting to individual employment. • Present information on the benefits of supported employment to local groups and businesses to expand supported employment in Kittitas County. • Provide technical assistance as needed to various public and private sector businesses in the County. a. Deliverable: WISE and/or a third party consultant shall organize and provide training and training materials as agreed upon by the County. Training expenses will be itemized and agreed upon prior to the provision of services. An invoice for training costs and related travel expenses shall be submitted to the County upon completion of services. Expenses are subject to the limitations outlined in the Special Terms and Conditions of this contract. Funding not to exceed $27,000 is provided for the following Individualized Technical Assistance activities and deliverables: 2. WISE and/or third party consultant shall provide assessment and consultation to the employment provider and client to identify and address existing barriers to employment. The maximum service hours per month the client can expect to receive is determined by the clients DDA Individual Service Plan (ISP). The County Service Authorization and updated Planned Rates information will not exceed the services outlined in the DDA ISP. Professional Services Agreement Page 13
