FUNDING AUTHORITY: Washington State Military Department (Department) and the U.S. Department of Homeland Security (DHS) 10. DESCRIPTION/JUSTIFICATION OF AMENDMENT: The Subrecipient experienced cost savings due to initial estimates for equipment being higher than the actual costs. To allocate the surplus , two new Program Areas are being added to enhance the Subrecipient's operational capacity (Program Area #6) and assist with damage assessments (Program Area #7). All additions to the Work Plan. Exhibit C, are denoted with grey highlight. There is no impact to the approved Budget, Exhibit E. To ensure full expenditure of the funding, the agreement is being extended one month. Deoartment Kev Personnel are being lJQdated as well. 11 . AMENDMENT TERMS AND CONDITIONS : 1. The overall agreement amount of $16,263 remains unchanged. 2. Change the Grant Agreement end date from August 31,2016 to September 30,2016. 3. Update the Article 1, Key Personnel Exhibit A as follows: a. Change Key Personnel from Deborah Henderson to Michael Alston, michael.alston@m ll,wa .gov, (253) 512-7083. 4. Replace original Work Plan Exhibit C with the attached revised Work Plan Exhibit C. 5. Replace original Milestones Exhibit D with the attached revised Milestones Exhibit D. This Amendment is incorporated in and made a part of the Grant Agreement. Except as amended herein, all other terms and conditions of the Grant Agreement remain in full force and effect. Any reference in the original Grant Agreement or an Amendment to the "Grant Agreement" shall mean 'Grant Agreement as amended". The Department and Subrecipient acknowledge and accept the terms of this Amendment as identified above, effective on the final date of execution below. By signing this Amendment, the signatories warrant they have the authority to execute this Amendment.
