PROFESSIONAL SERVICES AGREEMENT <br />BETWEEN KITTITAS COUNTY AND ELMVIEW <br />AMENDMENT #1 <br />PH DD 002 <br />The Professional Services Agreement between KITTITAS COUNTY ("County"), a <br />subdivision of the State of Washington, and ELMVIEW ("Contractor"), to provide a <br />coordinated and comprehensive local program of services for persons with disabilities, <br />attached hereto and incorporated by reference, is hereby amended to increase the <br />community access hourly service rate from $30.00 per hour to $32.00 per hour, effective <br />January 1,2017, as evidenced in the amended Attachment E below. <br />The amendment is mutually agreed to by the parties. <br />All other items in the original Agreement remain in effect. <br />Approved this 'iH' dayof-"-R~·~~b,-. __ ->,2017. <br />KITTIT AS COUNTY CONTRACTOR <br />BO .
