The SUBRECIPIENT shall submit a quarterly progress report describing current activities as outlined in the <br />Project Development Schedule (Attachment 4). <br />The SUBRECIPIENT shall submit a Final Report with final reimbursement no later than 45 days after <br />Agreement End Date. <br />The SUBRECIPIENT shall also comply with the Federal Funding Accountability and Transparency Act <br />(FFATA) and related OMB Guidance consistent with Public Law 109-282 as amended by section 6202(a) of <br />Public Law 110-252 (see 31 U.S.C . 6101 note) and complete and return to the Department the FFATA Form <br />located at http://mil.wa .govJemergency~management-division/grants/regu l redgrantforms and return to the <br />Department; wh ich is incorporated by reference and made a part of this Agreement. <br />TIME EXTENSIONS' <br />A time extension request for Agreement completion must be submitted by the SUBRECIPIENT to the <br />DEPARTMENT no later than 60 days before the end of the Period of Performance. A time extension request <br />must be in writing and identify the project, the reason the project' has not been completed within the approved <br />Period of Performance, a current status of the complet ion of the work, a detailed timeline for completion of the <br />remaining elements, and an anticipated completion date for the comp letion of the-remaining work . Failure to <br />timely submit a complete time extension request may result in denial of the time extension and loss of funding <br />for the project. <br />SUBRECIPIENT MONITORING: <br />The Department will monitor the activities of the SUBRECIPIENT from award to closeout. The goal 'of the <br />Department's monitoring activities will be to ensure that agencies rec::eiving federal pass-through fUnds are in <br />compliance with this Agreement, federal and state audit requirements, federal ·gran~ guidance, and applicable <br />federal and state financial regulations, as well as 2 CFR Part 200 Subpart F. <br />To document compliance with 2 CFR Part 200 Subpart F requirements, the SUBRECIPIENT shall complete <br />and return to the Department 2 CFR Part 200 Subpart F Audit Certification Form " located at <br />http://mil.wa .gov/emergencv-management-division/grantslreguiredgrantforms with the signed Agreement and <br />.each fiscal year thereafter until the Agreement is closed, which is Incorporated by reference and made a part of <br />this Agreement. <br />Monitoring activities may include, but are not limited to: <br />review of financial and performance reports; <br />monitoring and documenting the completion of Agreement deliverables; <br />documentation of phone calls, meetings, e-mails, and correspondence; <br />review of reimbursement requests and supporting documentation to ensure allowability and conSistency with <br />Agreement work plan, budget, and federal requirements; <br />observation and documentation of Agreement related activities, such as exercises, training, funded events, and <br />equipment demonstrations; <br />on-site visits to review equipment records and inventories, to verify source documentation for reimbursement <br />requests and performance reports, and to verify completion of deliverables. <br />The SUBRECIPIENT is required to meet or exceed the monitoring activities, as outlined above and in 2 CFR <br />Part 200, for any non-federal entity to which the SUBRECIPIENT makes a subaward as a pass-through entity <br />under this Agreement. <br />Compliancy will be monitored throughout the performance period to assess risk. Concerns will be addressed <br />through a Corrective Action Plan. <br />CLOSE-OUT <br />To initiate close-out, the SUBRECIPIENT is required to certify in writing the date completed and total amount <br />expended on the project on FINAL PROJECT REPORT form to the DEPARTMENT. After receipt of the FINAL <br />PROJECT REPORT form, the DEPARTMENT will conduct a site inspection and review supporting <br />documentation for compliance with the requirements of the Agreement. <br />Prior to project c lose-out , the SUBRECIPIENT shall provide the DEPARTMENT with acceptable <br />documentation slJPporting compliance with the AgreE;!ment.
