ATTACHMENT "A" <br />SCOPE OF WORK <br />Funding is provided for the following activities and deliverables: <br />1. Maintenance and updating of the developmental disability resource library. The <br />Contractor shall provide information on assistive devices, such as communication <br />boards, assistive software, and other technologies that increase a disabled <br />individual's independence. Contractor shall further operate a library to ensure <br />that adults and children with developmental disabilities in Kittitas County have <br />access to information and resources to support individuals with developmental <br />disabilities. <br />a. Deliverable: By June 30, 2017, the contractor will increase the amount of <br />informational materials that are current and up-to-date. <br />b. Deliverable: By June 30, 2017, the contractor will develop and implement <br />a system of on-site usage of assistive tablets. <br />2. Administration of transition workshops. Contractor shall provide transition <br />workshops to assist youth in seeking skills development to find employment as <br />well as understanding their rights and availabilities of resources. <br />a. Deliverable: By June 30, 2017, the contractor will organize at least twenty <br />transition workshops with at least ten participants each. <br />3. Provide quarterly reports to the County. <br />a. Quarterly reports to the County shall be due by the following dates: <br />i. October 15, 2016 (for July 1-September 30,2016) <br />I ii. January 15, 2017 (for October 1-December 31, 2016) <br />iii. April 15, 2017 (for January 1-March 31, 2017) <br />iv. July 15, 2017 (for April 1-June 30, 2017) <br />b. Quarterly reports shall include, at a minimum: <br />i. A narrative of progress on the deliverables outlined above. <br />ii. Number of individuals using the resource library. <br />iii. Log of resources and assistive devices checked out or utilized via the <br />resource library. <br />iv. Number of transition workshops held (include agendas or topics <br />discussed). <br />v. Number of participants attending each transition workshop. <br />4. Provide a report and/or presentation of progress of these deliverables to the <br />Kittitas County Mental Health/Developmental Disabilities Board prior to the end of <br />the contract period. <br />Professional Services Agreement <br />Page 13
