ATTACHMENT "A" <br />SCOPE OF WORK <br />Funding is provided for the following activities and deliverables: <br />1. Administer the Ages and Stages Questionnaire (ASQ) to aid in early identification of <br />children with developmental disabilities . <br />a. Deliverable : By June 30,2017, increase participation to an average minimum of <br />ten months per family per year. <br />b. Deliverable: By June 30, 2017 , provide the County with a report summarizing <br />participation and results in the ASQ program. <br />2. Conduct outreach activities to provide support, information, and assistance to families <br />and children with developmental disabilities. <br />a. Deliverable: By June 30, 2017 , conduct at least four outreach events. <br />3. Purchase appropriate manipulatives, books, and tools for play-based early intervention <br />services for the early interventionists to use during home visits and to build a lending <br />library for families with limited resources . <br />4. Purchase the evidence-based curriculum "It Takes Two to Talk" parent handbooks. <br />5. Provide staff training to improve services to children with developmental disabilities. <br />a. Deliverable: By June 30, 2017 , all educators will obtain at least fifteen hours <br />of professional development. <br />6 . Provide quarterly reports to the County . <br />a. Quarterly reports to the County shall be due by the following dates: <br />i. October 15, 2016 (for July 1-September 30,2016) <br />ii. January 15, 2017 (for October 1-December31 , 2016) <br />iii. April 15, 2017 (for January 1-March 31, 2017) <br />iv. July 15, 2017 (for April 1-June 30, 2017) <br />b. Quarterly reports shall include, at a minimum: <br />i. A narrative of progress on the deliverables outlined above. <br />ii. Number of outreach events attended. <br />iii . Number of children and families participating in the ASQ . <br />iv . Number of ASQ participants identified with a developmental <br />disability. <br />v. Number of staff attending training. <br />vi. Number of hours of professional development provided. <br />vii. A description of manipulatives/toys, books, and tools purchased . <br />viii. The number of It Takes Two to Talk Handbooks purchased. <br />7. Provide a report and/or presentation of progress of these deliverables to the Kittitas <br />County Mental Health/Developmental Disabilities Board or the Board of County <br />Commissioners prior to the end of the contract period. <br />Professional Services Agreement <br />Page 13
