The valid OMS control number for this information collection is 0596-0217. The time requiled to complete this <br />infomnatioo collection is estimated to average 3 hours per response, incfuding the time for reviewing instructions, searching existing data sources, gathering and <br />maintaining the data needed, and completing and reviewing the collection of information. <br />The U.S, Department of AgriculbJre (USDA) prohibits disaimination in all its programs and activities 011 the basis of race, color, national origin, age, disabiUty, and <br />where appficab!e, sex, marital status. famifial status, parental status, religion, sexual orientation, genetic infonnation, political be6efs, reprisal, 0( because all or <br />part of an individuars income is derived from any public assistance.
