Agreement between CWU and KCPHD Food Service Inspections

ATTACHMENT A The Contractor agrees to furnish the following materials and services to CWU: ln;pcctions and Inspection Reports Contractor will perform quarterly food services inspections of the following establishments: • North Village Cafe • Holmes Dining Room • Central Market Place • The catering kitchen on the 2"d floor of the SURC • 1891 Bistro • CWU food truck • CWU food wagon • Cat's Convenience Store • Cat Trax East • Cat Trax West • Coach's Coffee House • Jimmy B's • Breeze Thru Cafe Following each inspection, Contractor will submit an original Inspection Report, signed by the Unit Manager, or designee, for each establishment. The Unit Managers for each area are: • Geri Palmer -Nutt —North Village Cafd • Derek Smith — Holmes Dining Room, Central Market Place • Jim Matheny — The catering kitchen on the 2nd floor of the SDRC, 1891 Bistro, CWU food truck, CWU food wagon • Christine Cloninger — Cat's Convenience Store, Cat Trax East, Cat Trax West, Coach's Coffee House • Alison Mattson - Jimmy B's • Alex Lange — Breeze Thru Cafd Food Handler Permits Contractor agrees to provide upon request Washington State Food Handler Permit education and testing sessions for CWU employees. These sessions will be held on the CWU campus, with specific dates, times, and locations to be arranged at a future date between the parties. CWU employees participating in these sessions will be required to pay Contractor the required fees in order to obtain their Food Handler Permits. Hourly rate will not be charged to CWU for education and testing sessions.
