(3) Minerva Monitoring <br />The Contractor shall ensure that subcontractors have entered services funded under this <br />Contract in the Minerva. <br />(a) Ensure accurate and unduplicated reporting. <br />Contractor may not require subcontractor to enter duplicate prevention service data that is <br />entered into Minerva into an additional system. <br />(b) Ensure proper training of staff and designated back-up staff for Minerva data entry to meet <br />report due dates. <br />(4) Additional Monitoring Activities <br />The Contractor shall maintain records of additional monitoring activities in the Contractor's <br />subcontractor file and make them available to DSHS upon request including any audit and any <br />independent documentation. <br />d. Subcontractor Termination Requirements. <br />When terminating a subcontract, the Contractor shall withhold the final payment of any subcontract <br />until all required Minerva reporting is complete. This also applies to all subcontractor closures. <br />8.
