EXHIBIT "0" <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following; <br />1) Contractor shall recruit and coordinate with school advisors for the Youth for Truth <br />program. <br />a. Contractor shall provide copies of completed advisor contracts to the Kittitas <br />County Public Health Department (KCPHD) <br />2) Contractor shall provide evaluation information to KCPHD for project results/data <br />sheet publications. <br />3) Contractor shall coordinate with KCPHD to provide an end of year meeting with <br />Youth for Truth school advisors and teen mentor particpants. <br />4) Contractor shall complete all work by no later than June 1, 2016 <br />Professional Services Agreement <br />Page 13 <br />
