Agreement Number: K1862 <br />Page 3 of 8 <br /> <br />The Contractor shall provide a letter or evidence of completion of any work performed <br />with all invoices submitted which document satisfactory performance of the <br />Agreement. Before and after photos of the project are encouraged. The Department <br />will not pay invoices not accompanied by such documentation. All documentation <br />for final payment must be received no later than July 31, 2017 or the Contractor <br />will not be paid for the project discussed herein. Payment will not be provided <br />for any work conducted or purchases made after June 30, 2017. <br /> <br />Instead of one invoice, the Contractor may submit invoices, documentation of <br />satisfactory performance and proper receipts of expenses on a more frequent basis <br />provided it is not more than once a month. <br /> <br />6. DISALLOWED COSTS <br /> <br />The Contractor is responsible for any audit exceptions or disallowed costs incurred by <br />its own organization or that of its Subcontractors. <br /> <br />7. EVALUATION, REPORTING, AND MONITORING <br /> <br />a) The Contractor shall submit written documentation to the Department by October <br />31, 2016, confirming that the project will be completed by June 30, 2017, if the <br />project has not yet been completed. <br /> <br />b) The Contractor shall submit documentation to the Department when the project is <br />completed and confirming that the terms and conditions of this Agreement have <br />been met including a report of compliance with the Community Benefit provisions <br />of Section 3. <br /> <br />c) The Contractor shall cooperate with and freely participate in any monitoring or <br />evaluation activities conducted by the Department that are pertinent to the intent of <br />this Agreement, including an on-site inspection of the capital improvement project <br />after its completion and prior to June 30, 2017. <br /> <br />d) The Department or the State Auditor and any of their representatives shall have <br />full access to and the right to examine during normal business hours and as often <br />as the Department or the State Auditor may deem necessary, all the Contractor’s <br />records with respect to the matters covered in this Agreement. Such <br />representatives shall be permitted to audit, examine, and make excerpts or <br />transcripts from such records and to make audits of all contracts, invoices, <br />materials, payrolls, and records of matters covered by this Agreement. The <br />contractors shall retain records for a period of six (6) years following completion of <br />the project. <br /> <br />8. NONDISCRIMINATION PROVISION <br /> <br />a) During the performance of this Agreement, the Contractor shall abide by all <br />applicable federal and state nondiscrimination laws and regulations, including, but <br />not limited to chapter 49.60 RCW , Washington’s law against discrimination, and 42 <br />U.S.C. 12101 et. seq., the Americans with Disabilities Act (ADA). <br />
