FUNDING AUTHORITY: <br />Washington State Military Department (Department) and the Federal Emergency Management Agency (FEMA) <br />10. DESCRIPTION/JUSTIFICATION OF AMENDMENT, MODIFICATION, OR CHANGE ORDER: <br />The purpose of this Amendment is to revise the Project Development Schedule, Attachment 2, extending <br />the Grant Agreement End Date by 12 months. The applicant is requesting additional time due to <br />additional requirements and scheduling delays. The Department submitted a Period of Performance <br />extension request to FEMA for approval of the pending extension on November 3, 2016. This amendment <br />is contingent upon FEMA's approval of the time extension. <br />11. AMENDMENT TERMS AND CONDITIONS: <br />1. The original overall contract amount of $333,255.56 F, L; up to $249,941.67 F, $83,313.89 L, remains <br />unchanged. <br />2. Change the Period of Performance End Date from January 9, 2016 to January 9, 2017. <br />3. Replace the original Project Development Schedule, Attachment 2 with the revised Project Development <br />Schedule, attached. <br />4. No work can be done on this project after January 9, 2016, until FEMA approves the extension <br />request. Once an extension is approved by FEMA, work can continue on this project until January <br />9, 2017 or the date specified by FEMA. The county will be notified by either email or letter when <br />FEMA has made its decision. <br />This Amendment is incorporated in and made a part of the Grant Agreement. Except as amended herein, all other terms <br />and conditions of the Grant Agreement remain in full force and effect. Any reference in the original Grant Agreement or an <br />Amendment to the "Grant Agreement" shall mean "Grant Agreement as amended".
