The Subrecipient shall submit with each reimbursement request a report <br />indicating the Work Plan activities the expenditures for which <br />reimbursement is sought relate to, in the format provided by the <br />Department. <br />b. In conjunction with the next annual grant cycle application process, the <br />Subrecipient shall submit to the Department Key Personnel a final report <br />describing all completed activities under this Agreement. If a Subrecipient <br />will not be applying for grant funding during the next annual grant cycle <br />application process, the Subrecipient will submit a final report with its final <br />reimbursement request to the Department detailing progress on all <br />activities listed in the Work Plan. <br />c. In conjunction with the final report, the Subrecipient shall submit a separate <br />report detailing how the EMPG Exercise and Training requirements were <br />met for all personnel funded by federal or matching funds under this <br />Agreement. <br />d. The Subrecipient shall also comply with the Federal Funding Accountability <br />and Transparency Act (FFATA) and related OMB Guidance consistent with <br />Public Law 109-282 as amended by section 6202(a) of Public Law 110-252 <br />(see 31 U.S.C. 6101 note) and complete and return to the Department the <br />FFATA Form located at http://mil.wa.gov/emergency-management- <br />division/grants/requiredgrantforms, which is incorporated by reference and <br />made a part of this Agreement. <br />e. The Subrecipient shall participate in the State’s annual capabilities <br />assessment for the State Preparedness Report. <br /> <br />4. EQUIPMENT MANAGEMENT <br />a. Subrecipients and any non-federal entity to which the Subrecipient makes <br />a subaward shall comply with 2 CFR 200.318 – 200.326, to include but not <br />limited to: <br />i. Upon successful completion of the terms of this Agreement, all <br />equipment and supplies purchased through this Agreement will be <br />owned by the Subrecipient, or a recognized non-federal entity to <br />which the Subrecipient has made a subaward, for which a contract, <br />subrecipient grant agreement, or other means of legal transfer of <br />ownership is in place. <br />ii. All equipment, and supplies as applicable, purchased under this <br />Agreement will be recorded and maintained in the Subrecipient’s <br />inventory system. <br />iii. Equipment records shall include: a description of the property; the <br />manufacturer’s serial number, model number, or other identification <br />number; the source of the equipment, including the Federal Award <br />Identification Number (FAIN); Catalogue of Federal Domestic <br />Assistance (CFDA) number; who holds the title; the acquisition date; <br />the cost of the equipment and the percentage of Federal <br />participation in the cost; the location, use and condition of the <br />equipment at the date the information was reported; and disposition <br />data including the date of disposal and sale price of the property. <br />iv. The Subrecipient shall take a physical inventory of the equipment <br />and reconcile the results with the property records at least once <br />every two years.
