August 19, 2015 <br />INSTRUCTIONS FOR COMPLETING THE UNIFORM REPORT OF DBE AWARDS/COMMITMENTS AND PAS'MENTS <br />1. Indicate the DOT Operating Administration (GA) that provides your 8(H). The amounts in items 8(A) -9(p should include all apes of prime <br />contracts awarded and all types of surknnuacts awarded or committed. <br />including: professional or consul[am services, consone inn, purchase of <br />materials or supplies, lease or purchase of equipment and any other <br />types of services. All dollar amounts arc to reflect only the Federal <br />share of such contracts. and should be rounded to the nowed dollar <br />g(A). provide the total dollar amount for all prinne contracts assisted <br />with DOT Ponds that were awarded during this reporting period. <br />8(B), provide ncc Intal number of all prime contracts assisted with <br />DO'f funds that were awarded during this reporting period. <br />8(C). For all DBEs awarded prime contracts and awarlcd or <br />committed subcontracts as indicated in 8(C) -ID) and 9(C) -(D), break <br />the dant down further by total dollar amount as well as the number of <br />all contracts going (a each ethnic group as well as to non -minority <br />women. The "Other'- category includes those DBEs who arc not <br />members of the presumptively disadvantaged groups already bond, but <br />who arc determined eligible for life DBE program on an individual <br />basis (e.g, a Caucasian mule u'bll, a disability). The '1'O'I'ALS value in <br />1(RH) should equal the sum of 8(C) plus 9(C), and similarly', the <br />TOTALS value in I1(H) should equal the sun of 8(D) plus 9(D). <br />Column I should only be filled out if this report is due on December I, <br />as indicated in item 5.
