I=XHIRIT "A" <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following: <br />1) Contractor shall research and determine if past Community Assessments of <br />Neighborhood Stores Deliverables (CANS) assessments were completed in Kittitas <br />County communities in the past four years. <br />a. Contractor shall provide copies of past CANS assessment forms to the <br />Kittitas County Public Health Department (KCPHD). <br />2) Contractor shall provide training to assessment participants who will be conducting <br />the CANS assessments. <br />a. Contractor shall provide training sign -in sheets to KCPHD. <br />3) Contractor shall ensure assessment participants review the CANS "How to" <br />PowerPoint training prior to conducting CANS assessment activities. <br />a. Contractor shall provide training sign -in sheets to KCPHD. <br />4) Contractor shall complete CANS assessments for 20 — 30 tobacco retailers that <br />were assessed in the previous contract. <br />a. Contractor shall provide completed CANS assessments to KCPHD. <br />5) Contractor shall work with 1-2 tobacco retailers to: <br />a. Reduce the number of tobacco advertisements displayed in the tobacco <br />retail business; and <br />b. Have a "purchaser birth date" sign displayed at the cash register of the <br />tobacco retail business. <br />6) Contractor shall Recognize outstanding tobacco retailers and share CANS findings <br />through media releases and/or social media <br />7) Contractor shall present the CANS assessment findings to at least two (2) City <br />Councils, Boards of Health, or other group of elected officials. <br />8) Contractor shall communicate the CANS assessment findings to KCPHD for the <br />purpose of recognizing outstanding tobacco retailers and sharing CANS finding <br />through media releases and/or social media. <br />9) Contractor shall provide monthly progress reports to KCPHD using KCPHD <br />provided reporting templates. <br />Professional Services Agreement <br />Page 13 <br />
