I=XHIRIT "A" SCOPE OF WORK In addition to providing all material and labor, the Contractor shall perform the following: 1) Contractor shall research and determine if past Community Assessments of Neighborhood Stores Deliverables (CANS) assessments were completed in Kittitas County communities in the past four years. a. Contractor shall provide copies of past CANS assessment forms to the Kittitas County Public Health Department (KCPHD). 2) Contractor shall provide training to assessment participants who will be conducting the CANS assessments. a. Contractor shall provide training sign -in sheets to KCPHD. 3) Contractor shall ensure assessment participants review the CANS "How to" PowerPoint training prior to conducting CANS assessment activities. a. Contractor shall provide training sign -in sheets to KCPHD. 4) Contractor shall complete CANS assessments for 20 — 30 tobacco retailers that were assessed in the previous contract. a. Contractor shall provide completed CANS assessments to KCPHD. 5) Contractor shall work with 1-2 tobacco retailers to: a. Reduce the number of tobacco advertisements displayed in the tobacco retail business; and b. Have a "purchaser birth date" sign displayed at the cash register of the tobacco retail business. 6) Contractor shall Recognize outstanding tobacco retailers and share CANS findings through media releases and/or social media 7) Contractor shall present the CANS assessment findings to at least two (2) City Councils, Boards of Health, or other group of elected officials. 8) Contractor shall communicate the CANS assessment findings to KCPHD for the purpose of recognizing outstanding tobacco retailers and sharing CANS finding through media releases and/or social media. 9) Contractor shall provide monthly progress reports to KCPHD using KCPHD provided reporting templates.
