GCBH Agreement No. KITTITAS-MHBG-09110-00 <br />where these funds are expended. <br />(3) The Contractor shall submit an electronic MHBG project plan by April 20, 2010, to the GCBH <br />Contracts Coordinator, for GCBH's review and approval in accordance with the MHBG. This <br />project plan may be used to assist in development of the State's MHBG plan, and shall: <br />i. Specify the Criterion, activity description, number of consumers to be served, and a cost <br />basis for reasonableness of funding amount associated with activity; <br />ii. Be developed and based upon the Contractor's MHBG Fund award and/or reasonable <br />cost for the prior year; and <br />iii, Be developed and based upon the New Freedom Commission goals: <br />(A) Americans understand that mental health is essential to overall health. <br />(B) Mental health care is consumer and family driven. <br />(C) Disparities in mental health services are eliminated. <br />(D) Early mental screening, assessment, and referral to services are common practice. <br />(E) Excellent mental health care is delivered and researched is accelerated. <br />(F) Technology is used to access mental health care and information. <br />(3) The Contractor shall submit written MHBG progress reports, by the dates listed below, to the <br />GCBH Contracts Coordinator, using a BHR-provided report format, which shall detail the <br />planned or projected execution dates for each CG. In addition the Contractor shall report <br />how cultural competence has been incorporated into the work supported by mental health <br />block grant funds by providing a narrative that demonstrates the requirements of Section <br />5.a.(2) i - iii of this Agreement. <br />o December 20,2010; and <br />o March 20, 2010. <br />(4) The Contractor shall submit written MHBG implernentation reports, as described beiow, to <br />the GCBH Contracts Coordinator, which shall detail how the Contractor has met the CGs <br />identified in the MHBG Service Table in Section 8. In addition the Contractor shall report <br />how cultural competence has been incorporated into the work supported by mental health <br />block grant funds by providing a narrative that demonstrates the requirements of Section <br />5.a.(2) i - iii of this Agreement. <br />i. By July 20, 2010, the Contractor shall submit a written draft implementation report which <br />details: <br />o What CGs have been met and how they were met; <br />o Which CGs have not been met as of August 1, 2010; and <br />o How the Contractor shall meet unmet CGs, by September 30, 2010. <br />o How cultural competence has been incorporated into the work supported by mental <br />health block grant funds. <br />ii. By October 10, 2010, the Contractor shall submit a final written implementation report <br />which details how the Contractor has met all CGs identified in the MHBG Service Table <br />in Section 8, including any CGs detailed in any amendment(s) to this Agreement. <br />(5) The Contractor shall submit a written request to the GCBH Contracts Coordinator, using a <br />GCBH-provided form, for any requested changes to the MHBG Service Table in Section 8, no <br />later than 30 days prior to the proposed start date of requested changes. <br />6. MHBG Funding Requirements and Limitations. <br />a. The Contractor shall comply with the utilization guidelines contained in of the State's most recent <br />MHBG plan, accessible at http://wwwl.dshs.wa.gov/Mentalhealth/, which is signed by the <br />Washington State Governor, Specifically Title V, Section 1913 where the State agrees to comply <br />with the Public Health Service Act [42 U.S.C. 300x-1 et seq.]. <br />b. The Contractor shall not use MHBG Funds for the following: <br />(1) Services and programs that are covered under the capitation rate for Medicaid -covered <br />services to Medicaid enrollees. <br />(2) The Contractor's administrative costs associated with salaries and benefits at the <br />Contractor's organizational level. <br />-11- <br />
