Alcohol Drug Dependency Services 07-09 Contract Amendment #4 page 2 of 3 <br /> <br />This Contract between the Kittitas County and the Contractor is hereby amended as follows: <br /> <br />1. The AWARD AND REVENUES page, Exhibit A-3, is deleted and replaced by the attached revised <br />AWARD AND REVENUES page, Exhibit A-4. <br />2. The Federal – Sapt for Fiscal Year 2009 is increased by $6,996.06 from $129,889 to $136,885.06. <br />3. The State Grant-in-Aid for Fiscal Year 2009 is decreased by $7,868.00 from $143,496 to $135,628. <br />4. The maximum consideration for the entire contract is decreased by $871.94 from $781,343 to <br />$780,471.06. <br /> <br />All other terms and conditions of this Contract remain in full force and effect. <br />
