ATTACHMENT "A" <br />SCOPE OF WORK <br />Funding is provided for the following activities and deliverables: <br />1. WISE and/or third party consultant shall provide assessment and consultation to <br />the employment provider and client to identify and address existing barriers to <br />employment. The maximum service hours per month the client can expect to <br />receive is determined by the clients DDA Person -Centered Service Plan (PCSP). <br />The County Service Authorization and updated Planned Rates information will <br />not exceed the services outlined in the DDA PCSP. ITA services will not be <br />authorized beyond the months of July and August 2017 without further <br />amendment to the contract. <br />The ITA consultant shall develop and implement a plan for providing technical <br />assistance that supports in removing identified employment barriers. Planning <br />shall include all interested parties including the client, family members, friends, <br />employment providers, teachers, DDA Case Resource Managers, and any other <br />person identified by the client as being significant in their employment planning <br />process. This plan will incorporate clients' individual choices, reflect integration, <br />natural supports, and promote clients' individual rights and self-determination. <br />Plans will be reviewed and signed by the client and employment provider. <br />a. Deliverable: A copy of the Individual Technical Assistance plan and a <br />summary of the services indicated shall be provided to the County and <br />DDA Case Resource Manager. An invoice shall be submitted to the <br />County along with a monthly progress report. Reimbursement for ITA <br />services shall be paid with direct service dollars and shall not exceed <br />$12,250.00. Travel expenses associated with ITA services shall be <br />reimbursed with indirect service dollars and shall not exceed $1,000.00. <br />Professional Services Agreement <br />Page 13 <br />
