Attached is the draft plan/estimate that WISE submitted to us for the contract. Also, here is the <br />contract language we are proposing. Let me know if you have any other concerns I should consider <br />and how we will adjust our spending plan. Thank you! <br />SCOPE OF WORK <br />Funding not to exceed $18,000 is provided for the following Technical Assistance activities and <br />deliverables: <br />1. WISE and/or a third party consultant shall provide training and technical <br />assistance to employment providers and community members, including <br />families, employers, Developmental Disability Advisory Committee members, <br />school personnel, and/or other interested parties. Technical Assistance <br />training shall focus on the following Employment Development Activities: <br />Provide training to employment providers on job development, job <br />coaching, and transitioning clients from a group supported setting to <br />individual employment. <br />• Present information on the benefits of supported employment to local <br />groups and businesses to expand supported employment in Kittitas <br />County. <br />Provide technical assistance as needed to various public and private <br />sector businesses in the County. <br />a. Deliverable: WISE and/or a third party consultant shall organize and <br />provide training and training materials as agreed upon by the County. <br />Training expenses will be itemized and agreed upon prior to the <br />provision of services. An invoice for training costs and related travel <br />expenses shall be submitted to the County upon completion of services. <br />Expenses are subject to the limitations outlined in the Special Terms <br />and Conditions of this contract. <br />Funding not to exceed $27,000 is provided for the following Individualized Technical Assistance <br />activities and deliverables: <br />2. WISE and/or third party consultant shall provide assessment and consultation <br />to the employment provider and client to identify and address existing barriers <br />to employment. The maximum service hours per month the client can expect <br />to receive is determined by the clients DDA Individual Service Plan (ISP). The <br />County Service Authorization and updated Planned Rates information will not <br />exceed the services outlined in the DDA ISP. <br />The Individualized Technical Assistance consultant shall develop and implement a plan for <br />providing technical assistance that supports in removing identified employment barriers. <br />Planning shall include all interested parties including the client, family members, friends, <br />employment providers, teachers, DDA Case Resource Managers, and any other person <br />identified by the client as being significant in their employment planning process. This plan <br />will incorporate clients' individual choices, reflect integration, natural supports, and <br />promote clients' individual rights and self-determination.
