ATTACHMENT A <br />The Contractor agrees to furnish the following materials and services to CWU: <br />Inspections and Inspection Reports <br />1. Contractor will perform quarterly food services inspections of the following establishments: <br />• North Village Cafd <br />• Holmes Dining Room <br />• Central Market Place <br />• The catering kitchen on the 2 floor of the SURC <br />Cat's Convenience Store <br />• Cat Trax East <br />• Cat Trax West <br />• Coach's Coffee House <br />Jimmy B's <br />• Breeze Thru Cafd <br />• 1891 Bistro <br />• CWU food truck <br />• CWU food wagon <br />2. Following each inspection, Contractor will submit an original Inspection Report, signed by <br />the Unit Manager, or designee, for each establishment. The Unit Managers for each area are: <br />• Geri Palmer Nutt — North Village Cafd <br />• Derek Smith — Holmes Dining Room, Central Market Place <br />• Jim Matheny — The catering kitchen on the 2nd floor of the SDRC, 1891 Bistro, <br />CWU food truck, CAM food wagon <br />• Christine Cloninger — Cat's Convenience Store, Cat Trax East, Cat Trax West, <br />Coach's Coffee House <br />• Alison Mattson - Jimmy B's <br />• Alex Lange — Breeze Thru Cafd <br />Food Handler Permits <br />Contractor agrees to provide upon request Washington State Food Handler Permit education and testing <br />sessions for CWU employees. These sessions will be held on the CWU campus, with specific dates, <br />times, and locations to be arranged at a future date between the parties. CWU employees participating in <br />these sessions will be required to pay Contractor the required fees in order to obtain their Food Handler <br />Permits. Hourly rate will not be charged to CWU for education and testing sessions. <br />
