Request for Qualifications <br />Employment and Day Program Services <br /> <br /> <br />2017 Kittitas County RFQ 3 <br />Part I Qualifications Requirements <br />Section IA: General Information <br /> <br />1. Introduction <br />The purpose of this Request for Qualifications (RFQ) is to solicit new developmental disabilities Employment and <br />Day Program service providers (Contractors) who may qualify to be added to the Kittitas County Developmental <br />Disabilities Program Qualified Provider List. <br /> <br />Employment and Day Program Contractors currently working under Kittitas County Contract need not respond to <br />this RFQ. <br /> <br />Kittitas County contracts with qualified Employment and Day Program service providers, using funding from the <br />State of Washington Department of Social and Human Services (DSHS) and Developmental Disabilities <br />Administration (DDA) to provide the following Employment and Day Program Services: <br /> <br />• Individual Supported Employment <br />• Group Supported Employment <br />• Community Access <br /> <br />A description of each Employment and Day Program Service listed above is included in their respective Scope of <br />Work Section found in Section IB – Work Requirements of this RFQ. <br /> <br />Placement on the County Qualified Provider List does not guarantee a Contract will be offered to a Contractor. <br /> <br />2. Background <br />Pursuant to DDA guidelines, the County seeks to add qualified providers to its current Qualified Provider List, to <br />deliver the described services and which is used by authorized service recipients to procure applicable services. <br /> <br />3. Funding <br />Funding is determined by the number of individuals served by a Contractor and the service(s) provided. Current <br />Service Rates compensation for each Employment and Day Program Service is included in their respective Scope of <br />Work Section found in Section IB – Work Requirements in this RFQ. Rates are subject to change. <br /> <br />Section IB: Work Requirements <br /> <br />1. Accessibility <br />The Contractor shall provide means of access via phone, fax, and/or email at no cost to the individuals receiving <br />services. <br /> <br />2. Licenses and Certification <br />The Contractor shall maintain all applicable licenses and certifications required under DDA Policy 6.13 - Provider <br />Qualifications for Employment and Day Program Services: <br />https://www.dshs.wa.gov/sites/default/files/DDA/dda/documents/policy/policy6.13.pdf <br />
